1. Purpose

This SOP covers the installation, configuration, troubleshooting, and ongoing support of Voice over IP (VoIP) services at DTC-managed dental practices. DTC sells and supports two VoIP providers directly — Weave and Freedom Voice — but technicians will encounter a wide variety of third-party VoIP platforms and phone hardware at client sites. This document covers all scenarios: DTC-deployed systems, inherited third-party systems, and the network infrastructure that supports any VoIP deployment. It integrates DTC's UniFi network standards and establishes consistent procedures for all technician tiers.


2. Provider Landscape

2.1 DTC Partner Providers

These are the VoIP providers DTC sells, installs, and directly supports. Sections 5 and 6 contain full installation procedures for these providers.

2.1.1 Weave

Weave is a cloud VoIP service with integrated practice management software. It syncs directly with dental PMS platforms (Dentrix, Eaglesoft, Open Dental, etc.) to provide patient communication, team messaging, and analytics alongside phone service.

DTC's Role:

  • Install supporting network equipment
  • Install phones
  • Guide end users through basic orientation
  • DTC operates as a contractor — Weave handles direct client support, call tree programming, and software integration

Key Facts:

  • Client calls Weave directly for support (not DTC)
  • Weave contacts DTC only if a network issue is suspected
  • Weave software installs on the server (PMS data sync) and each workstation (patient notifications, team chat, SMS)
  • Weave bundles Yealink phones (typically T33G/T43U/T54W) — included with subscription (up to 5 free phone rentals, $4/month additional)
  • Higher cost — best ROI for larger practices or owners wanting integrated workflow
  • Weave now uses STUN for NAT traversal (in addition to server-side SIP rewriting)

2.1.2 Freedom Voice

Freedom Voice is a cloud VoIP service with a web-based PBX management system called Weblink. Simple, stable, and cost-effective. DTC uses Freedom Voice internally.

DTC's Role:

  • DTC is a Freedom Voice Partner — we manage every sold account with our own credentials
  • DTC provides Tier 1 support for Freedom Voice — most problems are resolved by us before escalating
  • We handle installs, configuration, and ongoing support

Key Facts:

  • Cheaper than traditional PSTN and cheaper than Weave
  • Last year Freedom Voice had only one outage that was not ISP-related
  • Freedom Voice uses server-side SIP header correction (rewrites SIP Address Header to Source IP) to work around NAT — does NOT use STUN/TURN/ICE

For Account Managers: When onboarding a new client with an existing third-party VoIP system, the first question is: does the client want to keep it? If yes, DTC supports the network — the provider supports the phones. If the client wants to migrate, scope the number porting timeline (typically 2-4 weeks), hardware needs (existing phones may or may not be reusable), and any contract termination fees with the current provider.

2.2.1 DTC Responsibility with Third-Party Providers

Regardless of which VoIP provider a client uses, DTC is responsible for:

  1. Network infrastructure — VLANs, switch ports, PoE, cabling, DHCP reservations
  2. Firewall / gateway — NAT configuration, IPS tuning, port access, SIP ALG disabled
  3. DNS — UDM as sole DNS server, no provider-specific DNS overrides (unless provider explicitly requires)
  4. Bandwidth — Ensuring adequate upload/download for concurrent calls alongside dental workflow
  5. Troubleshooting network-side issues — When provider says "it's your network," we need to prove or disprove that

DTC is NOT responsible for:

  • Provider portal configuration (call trees, extensions, voicemail)
  • Phone firmware or provisioning
  • PMS integration setup (unless it requires server/workstation software DTC manages)
  • Number porting logistics (coordinate with AM)

For Account Managers: When a client needs new phones, recommend Yealink T-series (T43U or T54W) as the default. They're the industry standard for SMB VoIP, have excellent NAT traversal, full encryption support, VLAN tagging, and are compatible with virtually every cloud VoIP provider. Price point is reasonable (~$100-290 depending on model). Only recommend Poly Edge if the provider specifically requires or bundles them.

For Technicians: When you encounter legacy phones without ICE/STUN support (Polycom VVX, Cisco SPA, old Grandstream GXP), your firewall NAT configuration becomes significantly more critical. These phones have zero ability to self-correct NAT issues. Every setting in Section 4.1 must be verified. With modern phones (Yealink, Poly Edge, Grandstream GRP), you have more resilience because the phone and provider together can compensate for imperfect NAT — but the firewall should still be configured correctly.


3. VoIP Network Architecture

3.1 How VoIP Works (Crash Course)

VoIP uses two protocols working together:

SIP (Session Initiation Protocol) — Sets up, manages, and tears down the call. Think of it as the phone ringing and being answered. Uses UDP port 5060 (or encrypted SIP on other ports).

RTP (Real-Time Transport Protocol) — Carries the actual voice audio once the call is connected. Uses dynamic UDP ports (typically 10000-20000 range, varies by provider).

Why NAT Matters: Both SIP and RTP include IP addresses inside their packet headers. When traffic passes through a NAT device (firewall/router), the outer IP gets translated but the embedded SIP header IP does not — unless the device or provider corrects it. This mismatch is the #1 cause of VoIP problems.

3.2 VLAN Placement

Standard Reference: VLAN architecture, naming, and isolation rules are defined in Pillars of Technology — Network Architecture. This document follows those standards.

Per DTC Network Standards, VoIP devices can be handled two ways:

Option A — CORP VLAN (Default) Place phones on the CORP VLAN alongside workstations and printers. This is the standard approach for most dental offices where phone count is low (under 10 phones) and bandwidth is adequate.

Option B — Dedicated VOIP VLAN (Recommended for Larger Installs) Create a separate VOIP VLAN per DTC Network Standards ("Additional VLANs should be created based on client requirements"). This is recommended when:

  • Office has 10+ phones
  • Bandwidth is limited or shared with heavy imaging traffic
  • QoS/traffic prioritization is needed
  • Client has compliance requirements for voice traffic isolation

Switch Port Configuration:

  • Phone ports = Access ports on VOIP VLAN, block all other VLANs
  • If phones have passthrough ports for workstations, the switch port must be a trunk port carrying both VOIP and CORP VLANs

3.3 IP Addressing for VoIP Endpoints

Per DTC Network Standards, use DHCP reservations for all VoIP endpoints. This ensures:

  • Consistent IP assignments for troubleshooting
  • Firewall rules can target phone IPs specifically
  • QoS policies can be applied by IP range

Document all phone MAC addresses and assigned IPs during installation.

3.4 DNS Configuration

Per DTC Network Standards: The UDM is the ONLY DNS server. Every phone — whether DHCP or static — must use the UDM gateway IP for DNS. No phone should point to a domain controller, external DNS, or provider-specific DNS.

VoIP phones resolve provider hostnames (e.g., Weave/Freedom Voice SIP servers) through the UDM's upstream DNS. No special DNS policies are needed for VoIP unless the provider specifies custom DNS requirements.


4. Pre-Installation Requirements

4.1 Network Readiness Checklist

Before installing any VoIP system, verify the following. This checklist applies to both providers.

⚠️ Engineer's Note: The old guidance was "avoid double NAT at all costs." Reality check: Comcast routers/modems do NOT use symmetrical NAT, so double NAT through Comcast equipment works fine for VoIP. The real problem is symmetrical NAT, not double NAT itself. If you're troubleshooting VoIP issues, find the actual problem first — don't reflexively blame double NAT.

Comcast Gateway Caveat: Bridge mode behavior varies by gateway model. Older Arris units handle bridge mode straightforwardly. Newer xFi gateways (XB6, XB7, XB8) may place the bridged network on a separate VLAN internally, which can introduce quirks. If bridging an xFi gateway, verify the WAN interface on the firewall is actually receiving the public IP and that no additional NAT layer is hiding. When in doubt, request a standalone modem from Comcast or use DMZ/1:1 NAT to the firewall as an alternative to bridge mode.

Getting a static IP should be a secondary step — after you've identified the actual root cause. A static IP is good practice but is not a magic fix for VoIP issues.

Rule of Thumb: For a dental office with 5 phones, assume 3 concurrent calls maximum = 300 Kbps dedicated to VoIP (G.711). This is trivial on any modern connection, but becomes relevant when:

  • Backup jobs are running (Veeam BDR replication)
  • Large imaging files are being transferred
  • Multiple workstations are streaming or updating

If bandwidth contention is suspected, check backup scheduling first — this is the most common bandwidth hog in dental offices.


5. Freedom Voice Installation

5.1 Pre-Install

  1. Review project scope and confirm phone count, extension assignments, and call tree requirements with Account Manager
  2. Complete network readiness checklist (Section 4.1)
  3. Stage phones and any required PoE injectors/switches
  4. Confirm number porting timeline with Freedom Voice — set up call forwarding from previous carrier if needed to ensure calls work during transition

5.2 Firewall Configuration

UniFi UDM (Standard):

  • No special VoIP-specific firewall changes required on UDM
  • UDM's default NAT behavior is VoIP-friendly
  • Verify IPS is enabled on all VLANs per DTC standard
  • If a dedicated VOIP VLAN is created, ensure IPS covers it
  • Create DHCP reservations for all phone MAC addresses

Call Tree / IVR Setup:

  • Call tree logic may vary by client — confirm with Account Manager

5.4 Phone Deployment

  1. Connect phones to designated switch ports (PoE or with power adapter)
  2. Phones should auto-register with Freedom Voice — verify registration in Weblink
  3. If phone activation fails, contact Freedom Voice — this is a provider-side issue
  4. Verify caller ID displays correctly (may be delayed if number porting is in progress)
  5. Test: make call, receive call, verify audio quality

5.5 Faxing

Freedom Voice supports faxing via Weblink:

Known Issue: Some dental clients experience ongoing fax issues with certain insurance providers. If faxing is consistently unreliable:

  1. Diagnose with Freedom Voice support first
  2. If unresolved, recommend a third-party fax service to the Account Manager:

5.6 Freedom Voice Encrypted SIP

Freedom Voice uses encrypted SIP over UDP. This bypasses all firewalls that do not perform SSL/TLS inspection. Encrypted SIP may need to be enabled by Freedom Voice — otherwise plain-text SIP is used by default.

If a firewall is performing SSL inspection and VoIP is failing, this is a likely cause. Encrypted SIP traffic will not decrypt properly through SSL inspection, causing session failures.


6. Weave Installation

6.1 Pre-Install

  1. Weave coordinates installs with DTC's Project Manager
  2. Complete network readiness checklist (Section 4.1)
  3. Stage phones and network equipment
  4. Weave handles call tree programming directly with the client
  5. Confirm number porting timeline and interim call forwarding

6.2 Firewall Configuration

Identical to Freedom Voice — see Section 5.2. Same UDM settings apply.

6.3 Software Installation

Weave requires software on both the server and workstations:

Server:

  • Weave sync agent — syncs practice management data to Weave's datacenter
  • Must have internet access (HTTPS outbound)
  • Installed on the PMS server (or a VM with PMS database access)

Workstations:

  • Weave desktop app — patient notifications, team messaging, SMS communication
  • Installed on each workstation that needs Weave integration

Note: Weave software installation and PMS integration configuration is typically handled by Weave's team. DTC's role is ensuring the network infrastructure supports it.

6.4 Phone Deployment

Same as Freedom Voice (Section 5.4), except:

  • Weave handles phone provisioning and configuration
  • DTC handles physical installation and network connectivity
  • Weave communicates directly with the client on feature setup

6.5 Post-Install

  • Confirm Weave is in communication with the client about what Weave handles vs. what DTC handles
  • Follow up with client to resolve any lingering network-related issues
  • Weave will contact DTC if they suspect network problems

7. Third-Party VoIP — Network Support

When DTC onboards a client that has an existing VoIP provider (not Weave or Freedom Voice), our role is ensuring the network supports it. This section covers what to do.

7.1 Network Assessment for Existing VoIP

During client onboarding or network assessment, document the following:

7.2 Network Cutover with Existing VoIP

When migrating a client's network (e.g., legacy firewall → UDM, flat network → VLAN segmentation) while keeping their existing VoIP provider:

  1. Before cutover: Document all phone IPs, MAC addresses, DHCP reservations, and any provider-specific network requirements
  2. During cutover:
    • Recreate DHCP reservations in UDM for all phones
    • Place phones on correct VLAN (CORP or dedicated VOIP)
    • Ensure switch ports are configured correctly (access ports for phones, trunk if passthrough)
    • Verify UDM IPS covers the VOIP VLAN
  3. After cutover: Run full validation (Section 8) — make calls, receive calls, verify two-way audio
  4. If issues arise: The troubleshooting in Section 9 is provider-agnostic — use it regardless of which VoIP provider the client has. Escalate to the provider if network-side checks are clean.

7.3 When the Provider Says "It's Your Network"

This is the most common escalation pattern. The provider blames the network, the client calls DTC. Here's how to handle it systematically:

  1. Run a speed test from the VOIP VLAN — document upload/download/jitter/packet loss
  2. Check IPS/firewall logs for any blocked VoIP traffic (SIP or RTP ports)
  3. Verify NAT configuration — SIP ALG off, no symmetrical NAT in path
  4. Check for bandwidth contention — is a backup running? Large file transfer? Windows updates?
  5. Run a packet capture (T3 only) — capture SIP REGISTER and a test call to verify signaling is clean from our side

If all five checks are clean, the issue is not our network. Document your findings and send them to the provider. Include: speed test results, firewall logs showing no blocks, NAT configuration screenshots, and packet capture summary if performed. This gives the provider concrete evidence and prevents the finger-pointing loop.


8. Testing & Validation

Perform these tests after every VoIP installation or network change that affects VoIP.

9. Troubleshooting

9.1 The Golden Rule

First, find the real problem.

Do NOT start by changing NAT settings, disabling firewall features, or requesting a static IP. Those are solutions — and you don't apply solutions until you've identified the problem.

Diagnostic Order:

  1. Identify the symptom — What exactly is happening? (no audio, one-way audio, dropped calls, registration failure, poor quality)
  2. Isolate the scope — All phones? One phone? All calls? Calls to specific numbers? Started when?
  3. Check the obvious — Is the internet up? Is the provider having an outage? Was anything changed on the network recently?
  4. Then dig — Use the symptom-specific guides below

9.2 Symptom: No Audio / One-Way Audio

NAT Explained for Techs:

  • Port-Restricted NAT (Consistent NAT): This is what we want. The source port mapping stays the same regardless of destination. VoIP works because the SIP server can tell phones where to send RTP based on the consistent mapping.
  • Symmetrical NAT: This breaks VoIP. The source port mapping changes based on the destination, so the SIP server gives phones the wrong address/port for RTP. This is the NAT type that actually kills VoIP — not double NAT.
  • STUN: A protocol where a server tells a device what its public IP:port mapping is. Weave uses STUN. Freedom Voice does NOT.
  • Server-side SIP correction: Both Weave and Freedom Voice rewrite the SIP packet's address header to match the actual source IP they see. This handles most NAT situations without STUN/TURN.

9.3 Symptom: Poor Voice Quality / Choppy Audio

  • QoS is only effective if the bottleneck is on YOUR network — it does not help with ISP congestion

9.4 Symptom: Dropped Calls

Comcast Double NAT Note: Comcast residential/business gateways use port-restricted NAT (not symmetrical). Double NAT through Comcast equipment does not break VoIP. The important thing is that neither device in the chain is doing symmetrical NAT. Note that gateway behavior varies by model — newer xFi gateways (XB6/XB7/XB8) handle bridge mode differently than older Arris units and may introduce quirks (see Section 4.1 for details).

11.2 SIP ALG — Why It Must Be Disabled

SIP Application Layer Gateway (ALG) is a "helper" feature built into many firewalls that attempts to rewrite SIP packet headers to fix NAT issues. In practice, it almost always makes things worse because:

  1. It rewrites SIP headers that the VoIP provider's server-side correction has already fixed
  2. It can corrupt SIP packets, causing registration failures
  3. It interferes with encrypted SIP (TLS/SRTP)
  4. Every firewall vendor implements it differently, creating unpredictable behavior

Rule: SIP ALG off. Always. On every device in the path.

SIP ALG on Other Firewalls (Client-Managed or ISP Equipment):

If a client has ISP-provided equipment or a non-DTC firewall in the path, SIP ALG may be enabled by default. Common locations:

  • ISP routers/gateways — Often enabled by default, sometimes hidden in advanced settings
  • Consumer routers — Almost always enabled by default (Netgear, Linksys, TP-Link, etc.)
  • When troubleshooting VoIP at a client site, check every device in the network path for SIP ALG, not just the DTC-managed UDM

UDM Note: UniFi UDM does not have a traditional SIP ALG toggle. Its IPS engine handles SIP traffic inspection differently and does not perform destructive SIP header rewriting.

11.3 How VoIP Providers Handle NAT

Different providers use different NAT traversal strategies. Understanding which approach a provider uses helps you troubleshoot faster.

Approach 1: Server-Side SIP Header Correction

  • The provider's SIP server reads the actual source IP from the IP packet header and replaces the SIP Contact/Via headers with that real IP
  • The SIP server always knows the real public IP:port of the phone, regardless of what the phone thinks its IP is
  • This bypasses most NAT issues without requiring STUN/TURN on the phone itself
  • Used by: Freedom Voice, Weave (in addition to STUN)
  • Limitation: Still breaks behind symmetrical NAT because the RTP port mapping is unpredictable

Approach 2: STUN (Session Traversal Utilities for NAT)

  • The phone queries a STUN server to discover its own public IP:port mapping
  • The phone then uses the STUN response to correctly populate its own SIP Contact/Via headers and SDP (Session Description Protocol) with the real public address — this is the phone correcting itself, not STUN rewriting anything
  • Provides better resilience than server-side correction alone because the phone already knows its public mapping before the SIP server even sees the packet
  • Used by: Weave, RingCentral, 8x8, Nextiva, Vonage, most modern UCaaS platforms
  • Limitation: Does NOT work behind symmetrical NAT (the mapping changes per destination, so the STUN-discovered mapping won't match the mapping used to reach the SIP server)

Approach 3: ICE (Interactive Connectivity Establishment)

  • Combines STUN and TURN. The phone first tries STUN for a direct connection. If that fails, it falls back to TURN (relay through a media server)
  • TURN works even behind symmetrical NAT, but at a performance cost — voice media is relayed through an intermediary server, adding latency
  • TURN is always a last resort because relayed media degrades call quality
  • Used by: RingCentral, 8x8, Zoom Phone, Microsoft Teams, Dialpad, most enterprise-grade UCaaS
  • Limitation: TURN adds latency. If calls are being relayed, you'll notice a slight delay. Fixing the NAT configuration is always better than relying on TURN.

Approach 4: Proprietary Gateway

  • The provider uses their own hardware gateway between the LAN and the internet
  • The gateway handles all NAT traversal internally
  • Used by: Ooma (Ooma Telo gateway)
  • Limitation: Single point of failure. If the gateway dies, all phones are down.

  • For most dental offices on modern internet circuits (100+ Mbps), QoS is unnecessary
